      The enclosed Certificate of Qualification documents
      that I am the Administrator of the estate of my sister
      Margrit Meyer, who died in Detroit on December 22, 2009.

      I am applying for the death certificate of Harold Russell
      Atkinson who died in Windsor ON, on December 23, 2004,
      because Margrit Meyer had named Harold Atkinson
      beneficiary on her death of certain retirement accounts
      she owned.  The financial institutions holding these
      retirement accounts, Concentra Financial of Saskatoon, SK,
      and AARP of Atlanta GA (USA), require documentation of
      the death of Harold Atkinson before distributing the
      assets that belonged to my sister.
      
      For reasons I do not understand, you have denied my previous
      application for a death certificate for Harold Atkinson,
      and I am authorizing an additional credit card payment of $30.
      However, I suggest as a matter of equity you refund to me or
      credit me with the $44 I have already paid you.
