Dear Ms. Schramm, Nothing my medical records for Mrs. Shaw should occasion special privacy concerns. These records show that from April 24, 2004 through the date of my last examination on February 9, 2011, Mrs. Shaw's visual function exceeded the requirements of the Registry of Motor Vehicles for the issuance and renewal of driver's licenses. These records also show Mrs. Shaw who for many years adhered meticulously to a complex regimen of glaucoma therapy to be an unusually conscientious, responsible and careful individual. I attach my medical records from April 24, 2004 to the present, in two versions: Record A is the raw, unedited text as it is stored on the fixed disc drive of my computer. Since this primary version contains non-standard acronyms and abbreviations which I find convenient but which are not in general usage, I wrote a computer program to translate the original into a text easier to interpret.
