In case of medical emergency: I do not want to be resuscitated. I do not want assisted or artificial respiration. I do not want intubation of any kind. I do not want gastric intubation. I do not want tracheal intubation. I do not want intravenous fluids. I do not want intravenous medication except for the relief of pain. I do not want antibiotics or steroids orally, topically, or intravenously. I do not want any procedure to prevent or abate hemorrhage. I do not want surgical intervention of any kind. If I am in pain, I ask that I be given morphine or other narcotics sublingually¬ orally, intramuscularly or intravenously in such doses as to suppress all pain without regard to possible respiratory depression.
