through 8/29/2017. No reference to any insurance, no reference to
Medicare D. If you wish me to do so, I can scan the three pages and 
attach the pdf file to an e-mail. PLEASE ADVISE.

The dumpster was delivered, and I found it very easy to discard in it
the storm window frames. I have not yet thrown away any glass. If you
will advise me what sort and how mach glass, if any, you wish to keep, 
I can probably finish disposing of the replacement window debris tomorrow
if there is no rain.
