b) I believe the problems concerning your friend David Gonzales which you describe, to be difficult to the point of being insoluble.

c) I wish I were in a position as a physician to offer to help to ameliorate them. Advice is of no help, because the problems are too complex. Words cannot do them justice. Only a participant will be able to be of any benefit.

d) I am unable help because: 
i) I'm too deaf to communicate reliably; 
ii) it's been 60 years since I stopped practicing general medicine, and 
iii) I've forgotten much of what I knew, and I'm unfamiliar with recent changes in the professiional management of the illness.
